Estonia is undertaking an ambitious reform of health and social care, with plans to strengthen coordination between services, create new regional structures for integrated care, introduce care coordination and shared wellbeing plans, and move towards population health management and more outcome-focused financing. The reform responds to a challenge familiar across many European systems: how to move from separate services towards more proactive, coordinated and person-centred care.

This webinar will use the Estonian experience as a practical starting point for a wider conversation about integrated care reform across Estonia, the Baltic states, the Nordic region and Central and Eastern Europe. The Estonian Ministry of Social Affairs will introduce the direction of the Estonian reform, followed by short perspectives on working across digital health, health insurance, public health and service delivery.

This webinar is particularly intended for attendees from Estonia, Latvia, Lithuania, Finland and the wider Baltic, Nordic and Central and Eastern European region. 

The session will also introduce the International Conference on Integrated Care (ICIC27), taking place in Tallinn from 10–12 May 2027.
